Please see the instructions below for work-related injuries or illnesses.

Complete First Injury Report

Life-Threatening Illness or Injury

  • If the situation is life-threatening, call 507-389-2111 or 911.
  • If you are unable to drive, security will be notified, respond, and direct the ambulance to you.

Note: If you decline ambulance transport, a co-worker or supervisor cannot transport you. You may arrange transportation by cab, family member, or friend.

  • Tell the doctor/nurse your illness or injury may be work related.
  • Report your injury to the workers’ compensation coordinator in Human Resources at 507-389-2015.

Non-Life-Threatening Illness or Injury

  • Immediately notify your supervisor.
  • Your supervisor, security, and you can determine if medical attention is needed.

If you do not need medical attention at this time:

  • Report your injury to the workers’ compensation coordinator in Human Resources at 507-389-2015.
  • Check in with your supervisor within a couple of days. Share contact info so they can follow up daily until you are okay.
  • If your condition does not improve within three (3) days, contact the coordinator at 507-389-2015 and visit:
